Position Title: Health Educator- Healthy Living Through Environment, Policy & Clinical Care (EPICC) Program

Essential Job Functions and Duties:

·       Perform duties related to the coordination, implementation, and delivery of Healthy Living programs and strategies.
·       Conduct education and promotional activities throughout the health district.
·       Mobilize partnerships. 
·       Develop policies and plans.
·       Link people to services such as health care providers.
·       Other duties as assigned.

Skills/Requirements

·       Must have excellent interpersonal skills.
·       Required to be computer literate.
·       Should be able to type 35 words per minute.

In the event of an emergency, such as a natural disaster or public health emergency
response event, all employees will be required to participate with TriCounty Health Department in its role as an essential community resource. This may
require working a non-traditional work schedule or working outside normal assigned
duties during the incident and/or emergency.

Salary may be negotiable with experience. Job is located in Vernal. TriCounty Health Department is a tobacco-free workplace.

The policy analyst will work with the Tobacco Prevention and Control Program, the Utah Tobacco Free Alliance, local health districts, and strategic partners to lead policy, systems and environmental change initiatives. The policy specialist will: develop, plan, implement and evaluate efforts affecting specific health policy changes related to tobacco; gather information, collect data, conduct needs assessments, perform policy analysis, develop and prepare required evaluation and other reporting documents; contact, meet, and collaborate with specific stakeholders to impact changes related to public health policy; act as a resource to expand, develop and coordinate public health programs at the state level; chair policy related work groups; provide education on tobacco issues to policy makers. The policy specialist will function under limited supervision and have considerable discretion in judgment in projects and assignments. POSITION TITLE:  Executive Director
REPORTS TO:  Board of Trustees
                                                                                                                                               

POSITION SUMMARY: The mission of CHC is to improve access to medical and dental care for Utah County residents who are uninsured.  To that end the Executive Director provides leadership in achieving CHC philosophy and mission, strategic direction, goals and objectives, ensures the fiscal stability of the program, and responds to the trends in health care.  He/she is responsive to the needs of the consumer, and manages the staff and the programs.  He/she is familiar with public health services policies and requirements.  He/she demonstrates community-based networking and develops collaborative relationships with other health and welfare organizations.

The Board of Directors is looking for a dynamic, motivated individual to lead the board and staff in accomplishing the organization’s mission. The Executive Director is appointed by and responsible to the Board of Trustees as the Chief Executive Officer of the Corporation and is charged with providing overall leadership, direction and administration of the operations of the Community Health Connect (CHC) in accordance with policies established by the Board of Trustees.  The Executive Director has the authority to both employ and discharge personnel consistent with the Personnel Policies and Procedures of the CHC.
                                                                                                                                               

POSITION QUALIFICATIONS:

Education Required: Requires a minimum of a Bachelor's degree in a health-related field, accounting or business administration. Preference for a Masters degree in related field.

Experience Required:  Proven capabilities in the areas of health care management, fiscal management, marketing strategy, long-range planning, organization, self-motivation, time management, monitoring and evaluation, personnel management, program development and implementation, quality control, fiscal reporting and grant writing.  Has previous work experience focused on the social-economic and health care problems of low-income groups in the service delivery area and of Medicaid/Medicare beneficiaries.

Other Qualifications:  Prefer experience and familiarity with public health service funded programs. An understanding of relationships among primary, secondary and tertiary care providers (i.e. referral arrangements, contractual payment mechanisms.) Demonstrated experience with not-for-profit Boards of Trustees and an understanding of the principles of non-profit corporate governance. Ability to communicate effectively, in written and oral form, with staff, clients, community representatives, government agencies, other health care providers and the Board of Trustees.  

Compenstaion: Salary commensurate with experience. Full benefits package including medical, dental, retirement. Full time exempt position.

STD PREVENTION COORDINATOR - Closes 12/18/13

This position is responsible to provide technical assistance and consultation on sexually transmitted disease (STD) management and prevention to local health departments, private providers, and community based organizations. Maintains contact with local county health department STD personnel regarding quality assurance, provision of care, as well as the status and closure of all cases of gonorrhea, chlamydia and syphilis. Supervises subordinate staff and collaborates with Bureau of Epidemiology staff on STDs in the statewide database to ensure the database is up-to-date and reports are being entered accurately. Conducts STD educational presentations to schools, youth and adult corrections/detention centers, substance abuse treatment facilities, and other groups/settings as requested. Ensures the completion of STD program narratives and grants.
